EXPLICIT CONSENT STATEMENT OF THE DATA SUBJECT PURSUANT TO THE LAW ON THE PROTECTION OF PERSONAL DATA

I state, acknowledge and undertake that I have read this enlightenment text and permit in advance, have an explicit consent and have been informed that my personal data within the scope of the Law on The Protection of Personal Data No.6698 is collected, recorded, processed and stored by  “ Hepşen İşletme Koçluğu Yönetim Gelişim Hizmetleri Tic.Ltd.Şti in accordance with the  principles in the law.

With regard to the processing of my personal data and my sensitive personal data to be processed by “ Hepşen İşletme Koçluğu Yönetim Gelişim Hizmetleri Tic.Ltd.Şti in accordance with “The Law on the Protection of Personal Data” No.6698,  I  state, acknowledge and understate that I give consent to any kind of my personal data and my sensitive personal data including the data which identifies or does not  identify my identify  in spoken/written or electronic environments  is processed by  “ Hepşen İşletme Koçluğu Yönetim Gelişim Hizmetleri Tic.Ltd.Şti’ and  shared with other persons or organizations, which are necessary to share  within relevant legislations ,and personal and sensitive personal data, of which details are given below, and processed within the scope identified in  “The Law on the Protection of Personal Data” No.6698 and in other relevant legislations  

• Data which is evident that belongs to an identified or identifiable natural person, which is processed fully or partially automatically or non-automatically as the part of data registry system and in which there exists information related to the identity of the person; documents like identity card , passport and driving license, tax number, social security number, signature, vehicle registry  plate and similar documents containing name-surname, TR identity number, nationality. the name of the father or mother, the place of birth and gender

• Data such as phone number, address, email address, fax number, IP address which is evident that belong to an identified or identifiable person and which are fully or partially automatically or non-automatically as a part of data registry system.

• Personal data which is evident that belong to an identified or identifiable person, which are fully or partially automatically or non-automatically as a part of data registry system and which are related to records and documents taker when entering a physical space and while staying in the physical space; camera records, fingerprints, records taken at security points, etc…

• Data which is evident that belong to an identified or identifiable person and which are found in photo and camera records and documents have a characteristic of a copy of documents containing personal data

• Data stated in Article 16 of the Law on The Protection of Personal Data(for example membership of an association or union, biometric data etc.) , which is evident that belong to an identified or identifiable person, which are fully or partially automatically or non-automatically as a part of data registry system

• Personal data related any request made to ‘’Hepşen İşletme Koçluğu Yönetim Gelişim Hizmetleri Tic.Ltd.Şti’ye” and withdrawal and evaluation of consent, , which is evident that belong to an identified or identifiable person, which are fully or partially automatically or non-automatically as a part of data registry system

In addition, I state, acknowledge and undertake that the personal data that I share with “Hepşen İşletme Koçluğu Yönetim Gelişim Hizmetleri Tic.Ltd.Şti’’ are correct and up-to-date, and if there is a change on this information, I inform ’Hepşen İşletme Koçluğu Yönetim Gelişim Hizmetleri Tic.Ltd.Şti’’ about.
